(b) The qualified therapist duties include:<br />
(1) Providing treatment as ordered by the attending physician, dentist or<br />
podiatrist.<br />
(2) Assisting the physician in evaluating level of function.<br />
(3) Assisting in developing and updating the plan of treatment.<br />
(4) Observing, recording and reporting information on the patient's<br />
condition to the attending physician and in the patient's health record.<br />
(5) Advising, consulting and, when appropriate, instructing family and<br />
other agency personnel, in patient's therapy program.<br />
(6) Teaching and supervising other health personnel when appropriate.<br />
(7) Evaluating the home environment and making appropriate recommendation.<br />
(8) Participating in in-service education programs.<br />

- Assessing client needs or admission for service as appropriate;<br />
- Implementing a medical plan of care and revising as necessary;<br />
- Initiating appropriate preventive, therapeutic, and rehabilitative techniques<br />
according to the medial plan of care;<br />
- Educating the client and family regarding treatment modalities and use of<br />
equipment and devices;<br />
- Providing consultation to other health care professional;<br />
- Communicating with the physician and other health care professionals regarding<br />
changes in the clients needs;<br />
- Supervising therapy assistants and home attendants as appropriate; and<br />
- Preparing clinical notes.<br />

HFS 133.15 Therapy Services:<br />
1. Provision of Services: Occupational services provided directly by the home health<br />
agency or arranged for under s. HFS 133.19, shall be given in accordance with the<br />
plan of care developed under s. HFS 133.20. Individuals providing these services shall<br />
perform the duties under s. HFS 133.14 (2) (a), (c), (f), (h) and (i).<br />
2. Occupational therapy. If offered, occupational therapy shall be provided by an<br />
occupational therapist or by a qualified occupational therapy assistant under the<br />
supervision of a qualified occupational therapist.<br />

DEGREE OF TRAVEL<br />
Home visits. Periodic office meetings. Must have reliable transportation, current drivers<br />
license, and agency-required liability insurance.'<br />
<br />
1. Successful completion of an occupational therapy program at a school of<br />
occupational therapy approved by the American Occupational Therapy<br />
Association or the Council on Medical Education and Hospitals of American<br />
Medical Association or the committee on Allied Health Education and<br />
Accreditation of the American Medical Association.<br />
2. Current and unencumbered license to practice occupational therapy specific<br />
to that state the employee is assigned to work by the Company.<br />
3. 2-3 years of occupational therapy experience preferred.<br />
4. Current CPR certification.<br />
5. Must be familiar with general use and functions of the computer, such as,<br />
usernames and password concepts; internet; e-mail; navigation of computer<br />
desktop, including starting programs, using files, and windows, effectively<br />
use navigation buttons and tool bars; ability to self-manage online HR<br />
services and online training programs.'<br />

The occupational therapist shall be currently licensed in the state, pursuant to<br />
Chapter 468,F.S., with 2-3 years of experience in occupational therapy.<br />

2. Services must be supplied only by qualified personnel and under the supervision of a<br />
physician licensed to practice in this State. Qualifications include licensure, registration,<br />
