<strong>Social Worker</strong><br /> <br /> General job duties. Provides social work services including assessment, intervention, and education to meet the psychosocial needs of adult patients and/or their family. The social worker will identify and provide therapeutic interventions necessary to meet their holistic needs. The Social Work Therapist will communicate with patients, families, and staff to ensure development of a safe and appropriate continuing care plan in preparation for discharge from the hospital. The role assists patients and their families in resolving personal and environmental issues that would negatively impact their discharge plan of care, predispose an individual to illness, or interfere with obtaining maximum benefit from medical care.<br /> Equipment: Equipment utilized in the unit/dept. (Is there computerized charting, medication systems, etc.) Epic charting<br /> Minimum Competencies/Essential Duties and Responsibilities<br /> 1.Perform psychosocial assessment with in 72 hours of patient admission and document in EMR.<br /> 2. Obtains collateral information from patient families and significant other for treatment plan.<br /> 3. Develops treatment plan in collaboration with medical and mental health staff, using information from the psychosocial assessment.<br /> 4. Communicates and coordinates with the treatment team as needed regarding patients mental health status.<br /> 5. Coordinates patient care with other health care providers such as psychiatrists, psychologists, nurses, resource specialists, and social service agencies.<br /> 6. Leads group therapy and individual sessions and document outcome in the EMR.<br /> 7. Performs family sessions, when appropriate.<br /> 8. Conducts evaluations of patients who may need to be committed to a hospital or other facility.<br /> 9. Monitors patients progress throughout his or her treatment.<br /> 10. Explains various treatment options and plans to patients<br /> 11. Connects patients to relevant resources within and outside the treatment facility.<br /> 12. Coordinates safe and effective discharges for transition to a different treatment facility or home.<br /> 13. Provides guidance to the resource specialist position.<br /> 14. Prepares and facilitates treatment team meeting.<br /> 15. Prepares commitment court request (legal documents) and submit to the courts. Collaborates with the court staff, attorneys, and judges to schedule court hearing. Should attend hearings with patients.<br /> 16. Maintains partnerships with organizations, courts, etc. within the community to better serve patients.<br /> 17. Other job duties as assigned.<br /> QUALIFICATIONS<br /> Required skills and minimum years of experience. 2 years minimum, mental health social worker<br /> Specific licensures and certifications/registrations required. (ARRT, CRT, BLS, ACLS, etc) LCSW<br /> Shift or schedule requirements. (Will the employee ever work alone? Is call required? If so, how often? Are weekends required?) M-F 8-4:30 No call, No weekends or holidays.<br /> Bed size/number of rooms for this unit/dept. 17 beds<br /> Dress Code: Casual professional<br /> <br /> Name of Position Requirements<br /> Last 4 of SS?
