Case Management Qualifications

Case managers need a bachelor's degree in nursing or a related field and a valid RN license. Attention to detail, empathy and the ability to manage complex situations is essential for successful case management. You should have one to two years of recent experience in case management. Specific facilities and units may request additional certifications and requirements. 

Case Management Duties & Responsibilities

As a travel case manager, you'll work for a specific time at one location. You'll serve as a guide for patients and their families, ensuring seamless care transitions. Your responsibilities will encompass an array of duties, including:

  • Conducting comprehensive assessments to ascertain patients' unique needs and preferences and laying the groundwork for personalized care plans

  • Standing as an unwavering advocate for your patients, championing their rights and ensuring their voices are heard throughout their healthcare journey

  • Coordinating transitions between different healthcare settings, focusing on uninterrupted care delivery and reducing transition stress for patients and families

  • Empowering patients and families with the information they need to make informed decisions about their care, enhancing their autonomy and confidence
